Dr. Rachel Gougian, DO is a USCIS Civil Surgeon approved to perform immigration exams and certify form I-693 for individuals seeking permanent residency (Green Card). She is fluent in English and Spanish.
The examination includes all elements required by the USCIS, with bloodwork, vaccines and paperwork completed onsite. Signed paperwork can typically be picked up within 5 days of the initial visit.

- You should be prepared with your childhood immunization records.
- If you do not have records, the doctor will scan the Maryland and DC immunization databases for your records. We provide a complimentary lab draw for immunity to MMR, varicella and hepatitis B.
- If you are lacking a vaccine, you can get it at our clinic covered by insurance.
- If you need to pay out-of-pocket for vaccines, the following prices apply:
