Bring your government-issued photo ID (passport, employment authorization, license or state ID), vaccination records (if available), any relevant medical history documentation, and a credit/debit card for payment. For most patients, we print the current I-693 form for you in office. However, you can bring your own I-693 form if you want to complete pages 1-2 ahead of time.
The physical examination and lab sample collection itself typically takes about 30 minutes.
After the initial physical exam, you will need to wait 3-5 days for lab results.
Based on your lab results, including antibody testing to determine your vaccine immunity status, you may need to return for a second visit to receive immunizations.
If you return for vaccines, you can expect to receive your sealed copy of I-693 on the same day before you leave the clinic.
